Admission Process

Step 1

Refer the patient to the county Medicaid District Office and request a Pre-Admission Screening (PAS) for Nursing Home Placement.

The patient’s physician must complete a PA-4 form for a Special Care AIDS Unit and fax it to the Medicaid District Office in order for the patient to be approved for placement

Step 2

Within 72 hours of receiving the PAS form, the Medicaid District Office will send a nurse to complete a Pre-Admissions Screening to determine the patient's eligibility for placement at Broadway House for Continuing Care.

The completed PAS evaluation form should be faxed to the attention of:

Step 4

After the patient is approved for placement at Broadway House by the Medicaid District Office:

  • Complete the Broadway House application and fax it to one of the numbers above.

  • If the patient’s Medicaid eligibility is pending or he has SSI income only, contact the county welfare office to apply for institutional or traditional Medicaid.

  • Complete an application for the AIDS Drugs Distribution Program (ADDP). This step is essential because it will provide adequate funds for HIV medications for those patients awaiting Medicaid eligibility.

  • For patients with private insurance, all insurance companies will be required to provide a written agreement to pay Broadway House for the care of the patient. Daily rates will be provided to the insurance company by the Admissions Office at Broadway House.

  • All applications must be completed and submitted to Broadway House for Continuing Care before patients can be admitted.
Step 5

Consult with the patient and his or her family to ensure that they are aware of the referral to Broadway House and are in agreement with this decision.

Medical Information Medical Information may be obtained from the patients Visiting Nurse Service, Clinic, Hospital, or Private Physician.

Please Note Broadway House is not licensed as a psychiatric facility. Broadway House is a skilled nursing facility, which provides 24-hour sub-acute nursing care.

All information must be completed before a patient will be considered for admission. Broadway House usually has a waiting list. You will be informed of the acceptance or denial for admission. The accepted patient will be placed on the waiting list and you will be contacted when a bed becomes available.
